Pamm's House Forms & Rates Here are the forms I use. If you are a client of Pamm's House or a potiental client, feel free to print the forms and bring them with you. If you are a child care provider, feel free to change the information to suit your need and use these forms (please do not add unchanged forms to your website ). You will need the Adobe Reader. These forms are to be filled out and returned to Pamm Along with a Copy of Your Child's Shot Record Child's Biography Permission Form Pick Up Form Rates & Contracts Pamm's House has three contract options. All are subject to space availablity and full-time is prefered over part-time. Price includes diapers, baby wipes, meals & snacks, Parent's Choice Fromula, infant cereal and baby food. Full Time Care: Your child's spot is guarenteed. Your payment is due each Friday before 11am and is due no matter how much your child attends. You are welcome to bring your child Monday-Friday, 7:30am to 5:30pm. Full-Time Agreement Current rate: $125 per week Part Time Care: Your child is scheduled for four or less days during the week. Your weekly payment will be determined on how many days your child is scheduled to attend. We will discuss days and availability at your interview. Part Time Agreement Current rate: $30 per day Drop-In Care: Must be pre-arranged ahead of time. This option is for non-regular attendance. Payment is due BEFORE care is given.
